改良式前庭康复训练对痉挛型脑性瘫痪患儿粗大运动功能及肌张力的影响

Effect of modified vestibular rehabilitation training on gross motor function and muscle tension in children with spastic cerebral palsy

摘要: 目的 观察改良式前庭康复训练对痉挛型脑性瘫痪(简称脑瘫)患儿粗大运动功能及肌张力的影响,为痉挛型脑瘫患儿的康复治疗提供了一种新方法。方法 选取2021年11月至2022年5月就诊于郑州大学附属儿童医院康复医学中心的符合纳入标准的40例痉挛型脑瘫患儿随机分为观察组和对照组,每组各20例。两组均给予常规康复训练进行干预,观察组在进行常规康复训练的基础上辅以改良式前庭康复训练。于治疗前和治疗8周后,分别采用粗大运动功能评定量表-88(GMFM-88)、改良Ashworth量表(MAS)进行评估,同时进行表面肌电图(sEMG)检查,观察患儿被动活动状态下腓肠肌、内收肌表面的肌电信号并记录平均肌电值(AEMG)。结果 8周治疗后,GMFM-88评分、MAS评分、被动活动状态下腓肠肌、内收肌表面的AEMG较治疗前均有明显改善,差异有统计学意义(P<0.05);且治疗后,观察组的GMFM-88评分高于对照组,MAS评分、腓肠肌AEMG分值、内收肌AEMG分值低于对照组,差异均有统计学意义(P<0.05)。结论 在常规康复训练的基础上辅以改良式前庭康复训练可以有效改善痉挛型脑瘫患儿的粗大运动功能,降低患儿的肌张力,提高患儿的生活质量。

Abstract: ObjectiveTo observe the effect of modified vestibular rehabilitation training on gross motor function and muscle tension of children with spastic cerebral palsy,and to provide a new method for rehabilitation of children with spastic cerebral palsy.MethodsFrom November 2021 to May 2022,40 children with spastic cerebral palsy who met the inclusion criteria in the Rehabilitation Medical Center of Children ′s Hospital Affiliated to Zhengzhou University were randomly divided into observation group and control group,with 20 cases in each group.Both groups were given routine rehabilitation training for intervention,and the observation group was supplemented with modified vestibular rehabilitation training on the basis of routine rehabilitation training.The gross motor function measure-88(GMFM-88),modified Ashworth scale(MAS) and surface electromyography(sEMG) were used for evaluation before treatment and 8 weeks after treatment.The sEMG of the gastrocnemius and adductor muscles were observed and the average electromyogram(AEMG) was recorded.ResultsAfter 8 weeks of treatment,GMFM-88,MAS,and AEMG of the gastrocnemius and adductor surface in the passive state were significantly improved in the two groups(P<0.05).After treatment,the GMFM-88 score of the observation group was higher than that of the control group(P<0.05),and the MAS score,gastrocnemius AEMG score and adductor AEMG score were lower than those of the control group(P<0.05).ConclusionThe modified vestibular rehabilitation training based on conventional rehabilitation treatment can effectively improve gross motor function and reduce muscle tension in children with spastic cerebral palsy,and improve the quality of life of children
