难治性肺炎支原体肺炎患儿T细胞亚群、IgE和RANTES检测的意义

摘要: 目的 探讨难治性肺炎支原体肺炎(RMPP)患儿T细胞亚群、IgE和调节激活正常T细胞表达和分泌的细胞因子(RANTES)检测水平及其在RMPP发病机制中的作用。方法 选取2019年2月至2022年8月郑州大学附属儿童医院呼吸科收治的肺炎支原体肺炎患儿190例为研究对象,按疾病类别分为实验组(RMPP患儿)95例,(普通肺炎支原体肺炎患儿)95例,对照组(疑似气管狭窄而无肺部感染患儿)90例。采用流式细胞术检测患儿外周血T细胞亚群CD4+、CD8+ T细胞表达情况,酶联免疫吸附试验法测定其血清及肺泡灌洗液IgE、RANTES表达水平。结果 (1)实验组、普通组和对照组CD4+、CD8+ T细胞表达率比较差异无统计学意义(P>0.05),实验组CD4/CD8比值高于普通组及对照组,差异有统计学意义(P<0.05)。(2)实验组血清及肺泡灌洗液中IgE和RANTES表达水平均高于普通组及对照组,普通组血清及肺泡灌洗液中IgE和RANTES表达水平均高于对照组,差异均有统计学意义(P<0.05)。结论 T细胞亚群、IgE和RANTES均参与了RMPP的发病过程,且后两者可作为RMPP的早期检测指标,优化临床诊治方案。

Abstract: ObjectiveTo explore the levels of detection of T cell subsets,IgE and regulated upon activation normal T cell expressed and secreted(RANTES) in infants with refractory Mycoplasma pneumoniae pneumonia(RMPP) and their role in pathogenesis of RMPP.MethodsA total of 190 children with Mycoplasma pneumoniae pneumonia were chosen as research subjects,who were treated in children′s Hospital Affiliated to Zhengzhou University from Feb.2019 to Aug.2022.They were divided into experimental group (95 children with RMPP),pneumonia group (95 children with Mycoplasma pneumoniae pneumonia) and control group (90 children suspected with tracheostenosis but without pulmonary infection).The expression of CD4+,CD8+ T cells of T cell subsets of peripheral blood were detected by flow cytometry.The level of IgE and RANTES were assayed with ELISA technique in their serum and bronchoalveolar lavage fluid.Results(1)The differences in CD4+ and CD8+ T cell expression rates among experimental group,pneumonia group and control group were not statistically significant(P>0.05).The CD4/CD8 ratio in experimental group was higher than that in pneumonia group and control group (all P<0.05).(2) Level of IgE in serum and bronchoalveolar lavage fluid in the experimental group was much higher than that in pneumonia group(P<0.05) and control group(P<0.01) and it was higher in pneumonia group than in control group in their serum and bronchoalveolar lavage fluid(P<0.05).(3) Level of RANTES in the experimental group was much higher than that in pneumonia group(P<0.05) and control group(P<0.01) and it was higher in pneumonia group than in control group in their serum and bronchoalveolar lavage fluid(P<0.05).ConclusionT cell subsets,IgE and RANTES are involved in the pathogenesis of RMPP to the regimen of latter two can be used as early detection indicators of RMPP,to optimize the regimen of clinical diagnosis and treatment.
