益肾通督针刺疗法对痉挛型脑性瘫痪患儿运动功能、认知水平及血清神经因子水平的影响机制探讨

摘要: 目的 探究益肾通督针刺疗法在痉挛型脑性瘫痪(简称脑瘫)患儿中的应用效果,及其对运动功能、认知水平及血清神经因子水平的影响。方法 选取我科2019年8月至2021年9月收治痉挛型脑瘫患儿89例作为研究对象,随机分为两组,对照组44例给予运动疗法,观察组45例给予联合益肾通督针刺疗法治疗,对比两组患儿运动功能,认知功能及神经因子水平。结果 治疗后,观察组粗大运动功能、精细运动功能评分高于对照组(P<0.05);Gesell发育诊断量表评分中精细动作、语言、个人-社交评分高于对照组(P<0.05),大动作、适应性评分呈上升趋势,差异无统计学意义(P>0.05);观察组神经生长因子、脑源性神经营养因子水平高于对照组(P<0.05)。结论 益肾通督针刺法结合运动疗法可通过益肾健脾、通督疏经之功,以及调节血清神经生长因子及脑源性神经营养因子表达水平,有效提高粗大与精细运动功能,促进神经认知发育水平。

Abstract: ObjectiveTo explore the application effect of Yishen Tongdu acupuncture therapy in children with spastic cerebral palsy(CP), and its influence on motor function, cognitive level and serum neurofactor level.MethodsA total of 89 children with spastic cerebral palsy admitted to our department from August 2019 to September 2021 were selected as the research subjects and randomly divided into two groups. The control group(n=44) was given exercise therapy, and the observation group(n=45) was given exercise combined Yishen Tongdu acupuncture therapy. The motor function, cognitive function and neurofactor levels of the two groups were compared. ResultsAfter treatment, the gross motor function and fine motor function scores of the observation group were higher than those of the control group (P<0.05). Gesell Developmental Diagnostic Scale scores of fine motor and language,and personal-social scores of observation group were higher than those of the control group(P<0.05), and the scores of gross motor and adaptability tended to increase, and the differences were not statistically significant(P>0.05). The levels of nerve growth factor and brain-derived neurotrophic factor in the observation group were higher than those in the control group(P<0.05).ConclusionYishen Tongdu acupuncture combined with exercise therapy can effectively improve gross and fine motor function and promote neurocognitive development by strengthening kidney and spleen, Tongdu and dredging meridian, and regulating the expression levels of serum nerve growth factor and brain-derived neurotrophic factor.
