肺炎支原体肺炎患儿并发心肌损害的危险因素研究

摘要: 目的 探讨肺炎支原体肺炎(MPP)并发心肌损害的相关影响因素。方法 选取2018年1月至2020年1月就诊于潍坊市妇幼保健院儿科诊断为MPP的126例儿童作为研究对象,根据是否并发心肌损害分为观察组(MPP并发心肌损害)66例和对照组(无并发心肌损害)60例。收集研究对象的一般资料,观察热程、热度峰值、胸片结果、住院时间、发病年龄,了解影响MPP患儿并发心肌损害的主要影响因素。结果 (1)经单因素分析得出5个因素差异有统计学意义(P<0.05),热程>5 d、热度峰值>39℃、胸片示大叶性肺炎、住院时间>10 d、发病年龄<2岁;(2)Logistic回归分析得出热程>5 d、胸片示大叶性肺炎、发病年龄<2岁是影响MPP并发心肌损害的主要危险因素。结论 对肺炎支原体肺炎患儿存在发热>5 d,胸片提示大叶性肺炎、年龄<2岁的情况应注意并发心肌损害的可能;对MPP并发心肌损害的高危对象,应加强管理,完善心电图、心脏彩超等相关必要性检查,积极干预以改善MPP患儿预后。

Abstract:

ObjectiveTo investigate the related influencing factors of Mycoplasma pneumoniae pneumonia(MPP) complicated with myocardial damage.MethodsA total of 126 children diagnosed with MPP in the Department of Pediatrics of Weifang Maternal and Child Health Care Hospital from January 2018 to January 2020 were selected as the research subjects.According to whether they were complicated with myocardial injurythey were divided into observation group(MPP complicated with myocardial injury66 cases) and control group(without complication with myocardial injury60 cases).The general data of the subjects were collectedand the heat durationheat peakchest X-ray resultslength of hospital stayand age of onset were observed to learn about the main influencing factors of myocardial damage in children with MPP.Results(1) Univariate analysis showed that the difference in 5 factors was statistically significant (P<0.05):the heat course was >5 daysthe heat peak was >39 ℃,the chest film showed lobar pneumoniathe hospital stay was >10 daysand the onset age was <2 years.(2)Logistic regression analysis showed that heat duration >5 dayslobular pneumonia on chest radiograph and onset age <2 years were the main risk factors for MPP complicated with myocardial damage.ConclusionFor children with Mycoplasma pneumoniae pneumoniaif fever lasts more than 5 dayschest radiograph indicates lobar pneumoniaand the onset age is less than 2 yearsthe possibility of being complicated with myocardial damage should be considered.For patients at high risk of MPP complicated with myocardial damageit is necessary to strengthen managementcomplete the necessary examinations such as electrocardiography and cardiac color ultrasonographyand actively intervene to improve the prognosis of MPP children.
