川崎病与流感病毒感染的临床相关性探讨

摘要: 目的 通过分析川崎病合并流感病毒感染的临床特点,进一步提高临床医师对川崎病病因认识。方法 选取2010年6月至2019年12月我院住院治疗的川崎病患儿57例,依据是否合并流感病毒感染分为观察组19例和对照组38例。观察组为川崎病合并流感病毒感染者,对照组为川崎病不合并流感病毒感染者。比较分析两组的临床表现,实验室指标包括白细胞计数、中性粒细胞比例、血红蛋白、C反应蛋白(CRP)、血沉、丙氨酸转氨酶(ALT)、天冬氨酸转氨酶(AST)、肌酸激酶同工酶(CK-MB)水平。丙种球蛋白的治疗效果及冠状动脉受损情况。结果 观察组发热时间显著长于对照组,CRP、血沉、ALT、CK-MB水平高于对照组,差异均具有统计学意义(P<0.05)。川崎病合并流感患儿对首次丙种球蛋白无反应率、冠状动脉损害的发生率更高,住院时间更长。结论 川崎病的发生可能与流感病毒引起的免疫功能紊乱有关。

Abstract:

ObjectiveBy analyzing the clinical characteristics of Kawasaki disease(KD) complicated with influenza virus infection, to further improve the clinicians' understanding of the etiology of Kawasaki disease. MethodsA total of 57 children with Kawasaki disease hospitalized in our hospital from June 2010 to December 2019 were selected. Children with KD and influenza virus infection were selected as experiment group(n=19)and children with KD alone were selected as control group(n=38). The clinical manifestations and laboratory test indexes were compared, including white blood cell(WBC),hemoglobin(Hb),erythrocyte sedimentation rate(ESR),C-reactive protein(CRP),neutrophilic granulocyte percentage(N%),alanine aminotransferase(ALT),aspartate aminotransferase(AST),creatine kinase-MB(CK-MB).The therapeutic effect of intravenous immunoglobulin(IVIG) and incidence of coronary artery injury were also analyzed.ResultsFever days in experiment group were longer than those in control group. ESR,CRP,ALT and CK-MB levels in experiment group were all higher than those in control group,and the difference was statistically significant(P<0.05).The no-response rate to first-time IVIG, and the incidence of coronary artery injury in experiment group were higher than those in the control group.ConclusionThe occurrence of KD may be associated with immune dysfunction caused by influenza virus.
