摘要: 癫痫的防治策略已经由控制癫痫发作向控制癫痫发生前移。癫痫发生是癫痫的萌芽和前期阶段,是癫痫形成的“时间窗口期”,也是药物干预的最佳阶段。神经元损伤与免疫炎症是癫痫发生的基本特征,免疫炎症参与儿童癫痫发生的全过程。研究发现抑制早期炎症因子(白细胞介素-1、肿瘤坏死因子 α等)并不能阻止癫痫发生,近年来晚期炎症因子高迁移蛋白 B1(HMGB1)在癫痫发生中的特殊地位越来越被重视。在功能上,HMGB1存在细胞内基因调控和细胞外促炎的两种不同功能。抑制 HMGB1的转胞易位是调控其功 能 的 关 键。HMGB1 通 过 两 个 经 典 的 转 导 通 路 激 活 下 游 免 疫 反 应:HMGB1-RAGE 轴 和HMGB1-TLRs轴。中药主要通过 HMGB1/TRL4/NF-κB通路调节免疫炎症反应干预癫痫发生的进程。本文从 HMGB1的源流、结构、功能、释放形式、信号通路、抑制剂及中医药干预的概况进行综述,为中医药干预 HMGB1及癫痫发生提供文献依据。

Abstract:

The prevention and treatment strategy of epilepsy has shifted from controlling epileptic seizures to controlling epileptogenesis. Epileptogenesis is the budding and early stage of child epilepsy, the "time window period" of epilepsy formation, and the best stage of drug intervention. Neuronal damage and immune inflammation are the basic characteristics of epileptogenesis, and immune inflammation participates in the whole process of epileptogenesis in children. Studies have found that inhibiting early inflammatory factors(IL-1, TNF, etc.) cannot prevent epileptogenesis. In recent years, the special position of high mobility protein B1(HMGB1) of late inflammatory factor in immune inflammation has been paid more and more attention. Functionally, HMGB1 has two different functions: intracellular gene regulation and extracellular pro-inflammation. Inhibiting the translocation of HMGB1 is the key to regulating its function. HMGB1 activates the downstream immune response through two classic transduction pathways: the HMGB1-RAGE axis and the HMGB1-TLRs axis. Traditional Chinese medicine mainly regulates the immune inflammatory response and interferes with the process of epilepsy through the HMGB1/TRL4/NFB pathway. This article reviews the sources, structures, functions, release forms, signal pathways, inhibitors and traditional Chinese medicine interventions of HMGB1 in order to provide a literature basis for traditional Chinese medicine intervention in HMGB1 and epileptogenesis.
