摘要: 目的 探讨脑性瘫痪(简称脑瘫)患儿营养不良的相关因素。方法 采用Z评分法回顾性分析2017年10月至2020年5月深圳市罗湖妇幼保健院52例脑瘫患儿的营养情况,并采用Logistic回归分析家庭因素、孕时产时因素、产后因素、喂养情况、疾病情况等因素与脑瘫患儿营养不良做相关分析,以筛选出与脑瘫患儿营养不良相关的危险因素。结果 52例脑瘫患儿,其中营养不良27例(51.9%)。单因素分析发现,父母生育年龄、吞咽障碍、粗大运动功能分级系统(GMFCS)评分均是脑瘫患儿营养不良的危险因素,差异均有统计学意义(P<0.05)。多因素Logistic回归分析发现,吞咽障碍(OR=5.093,95%CI=1.132~22.905)、GMFCS(OR=1.930,95%CI=1.209~3.079)是脑瘫患儿营养不良的独立危险因素。结论 脑瘫患儿营养不良发生率较高,可根据脑瘫患儿营养不良的高危因素情况,制定相应干预措施,从而改善脑瘫患儿营养健康状况。

Abstract:

ObjectiveTo explore the related factors of malnutrition in children with cerebral palsy.MethodsThe Z-scoring method was used to retrospectively analyze the nutritional status of 52 inpatients with cerebral palsy treated in Shenzhen Luohu Maternity and Child Health Hospital from October 2017 to May 2020. Screen out the related factors of malnutrition in hospitalized children with cerebral palsy by using Logistic regression analysis to analyze the correlation between family factors, factors during pregnancy and delivery, postpartum factors, feeding conditions and disease conditions.ResultsAmong the 52 children with cerebral palsy, 27 were malnourished. The incidence of malnutrition was 51.9%. Univariate analysis showed that parents' reproductive age, dysphagia, and GMFCS scores were risk factors for malnutrition in children with cerebral palsy,and the differences were statistically significant(P<0.05). Multivariate Logistic regression analysis showed that dysphagia OR=5.09395%CI=1.132-22.905and GMFCSOR=1.93095%CI=1.209-3.079were independent risk factors for malnutrition in children with cerebral palsy. ConclusionThe incidence of malnutrition in children with cerebral palsy is relatively high. Based on the high-risk factors of malnutrition in children with cerebral palsy, corresponding intervention

 measures can be formulated to improve the nutritional health of children with cerebral palsy.
