摘要: 目的 探究多种感觉刺激对病毒性脑炎伴吞咽功能障碍患儿吞咽功能及生活质量的影响。方法 选取2018年7月至2020年9月我科收治的伴吞咽障碍的病毒性脑炎患儿109例为研究对象,随机分为观察组55例和对照组54例。对照组给予常规护理及直接摄食训练,观察组在对照组基础上采用冷刺激、手法机械刺激、主被动训练等多种感觉刺激,20 d后观察两组患儿干预前后的吞咽障碍改善情况、吞咽功能及生活质量结果 观察组患儿吞咽障碍改善情况总有效率为94.55%(52/55),高于对照组79.63%(43/54),差异有统计学意义(P<0.05)。观察组SSA和DDS评分低于对照组,儿童生活质量普通适用核心量表各项评分均高于对照组,差异有统计学意义(P<0.05)。结论 多种感觉刺激对病毒性脑炎伴吞咽功能障碍患儿治疗效果确切,能够通过兴奋感觉神经纤维,提高咽部反射能力,缓解肌群紧张等机制改善吞咽功能障碍,增强吞咽功能,提高生活质量。

Abstract:

ObjectiveTo investigate the effects of multiple sensory stimulation on swallowing function and quality of life in children with viral encephalitis and dysphagia.MethodsA total of 109 children with viral encephalitis with dysphagia admitted to our department from July 2018 to September 2020 were selected as the research subjects, and they were randomly divided into observation group(n=55) and control group(n=54). The control group was given routine nursing and direct feeding training, and the observation group was given cold stimulation, manual mechanical stimulation, active and passive training and other sensory stimulation in addition to those given tothe control group. After 20 days, the improvement in swallowing disorders, swallowing function and quality of life in the two groups before and after intervention were observed.ResultsThe total effective rate of improvement in swallowing function in the observation group was 94.55%(52/55), which was higher than that in the control group(79.63%43/54), and the difference was statistically significant(P<0.05). The scores of SSA and DDS in the observation group were lower than those in the control group, and the scores of the common applicable core scale for children's quality of life were higher than those in the control group, there being statistical significance(P<0.05).ConclusionMultiple sensory stimulation has a definite therapeutic effect on children with viral encephalitis and dysphagia, which can improve dysphagia, enhance swallowing function and improve quality of life by stimulating sensory nerve fibers, improving pharyngeal reflex ability,and relieving muscle tension and other mechanisms.
