麻杏石甘汤联合西药治疗儿童社区获得性肺炎疗效的Meta分析

Meta-analysis of the efficacy of Maxing Shigan decoction combined with western medicine in the treatment of community-acquired pneumonia in children

摘要: 目的 系统评价麻杏石甘汤联合西医常规治疗儿童肺炎的疗效及安全性。方法 计算机检索国内外各数据库自建库至2023年8月1日所发表的联用麻杏石甘汤治疗儿童肺炎的随机对照试验,2名研究者单独进行文献筛选与资料提取,并对纳入研究进行质量评价,采用RevMan 5.3软件进行Meta分析。结果 共纳入9项研究,共涉及1 005例患者,其中试验组504例,对照组501例。Meta分析结果表明联用麻杏石甘汤治疗小儿肺炎的总有效率[RR=1.16,95%CI(1.11,1.22),P<0.000 01]、咳嗽消失时间[SMD=-1.60,95%CI(-1.81,-1.39),P<0.000 01]、退热时间[SMD=-1.43,95%CI(-2.04,-0.82),P<0.000 01]、肺部啰音消失时间[SMD=-1.35,95%CI(-1.55,-1.15),P<0.000 01]、治疗后血清C反应蛋白水平均优于对照组,不良反应及并发症发生情况优于对照组[RR=0.41,95%CI(0.18,0.95),P=0.04]。结论 麻杏石甘汤联合西医常规治疗儿童肺炎疗效确切,安全性优于西医常规治疗,但尚需高质量随机对照试验研究进一步验证。

Abstract: ObjectiveTo systematically evaluate the efficacy and safety of Maxing Shigan decoction combined withconventional western medicine in the treatment of pneumonia in children.MethodsRandomized controlled trials of combined use of Maxing Shigan decoction in the treatment of childhood pneumonia were searched by computer in the databases at home and abroad, which were published from the establishment of databases to August 1, 2023. Two researchers conducted literature screening and data extraction separately, and the quality of the included studies was evaluated. Meta-analysis was performed using RevMan 5.3 software.ResultsA total of 9 studies were included, involving a total of 1 005 patients, including 504 patients in the experimental group and 501 patients in the control group. The results of Meta-analysis showed that the total effective rate of combined use of Maxing Shigan decoction in the treatment of children with pneumonia[RR1.1695%CI1.11,1.22,P<0.000 01, the time of cough disappearanceSMD=-1.60,95%CI (-1.81,-1.39),P<0.000 01, fever relief timeSMD=-1.43,95%CI (-2.04,-0.82),P<0.000 01, lung rale disappearance timeSMD-1.35,95%CI-1.55,-1.15,P<0.000 01, and the levels of serum CRP after treatment in the experimental group were better than those of control group, and the incidence of adverse reactions and complications was better than that of control groupRR=0.41,95%CI(0.18,0.95)P=0.04.ConclusionMaxing Shigan decoction combined with conventional western medicine is effective, and is also safer than conventional western medicine in the treatment of children with pneumonia, but high-quality RCT studies are needed for further verification.
