Clinical characteristics of children with Henoch-Sehonlein purpura of abdominal type and the application value of endoscopic diagnosis under narrow band imaging

Abstract: Objective:To analyze the clinical characteristics of children with Henoch-Sehonlein purpura(HSP) of abdominal type and the application value of NBI endoscopy in the diagnosis and treatment of HSP of abdominal type.
Methods:Seventy children with suspected HSP of abdominal type who were admitted to the hospital between May 2016 and May 2019 were selected as the subjects. They were examined with abdominal B ultrasound and NBI endoscopy. The clinical characteristics of children with HSP were summarized. Based on the pathological diagnosis results, the diagnosis rates of HSP of abdominal type by the two examination methods were compared. The application value of NBI endoscopy in the clinical diagnosis of children with HSP of abdominal type was analyzed.
Results:In this study, all subjects had different degrees of abdominal pain, with or without paroxysmal exacerbation. The pain was not fixed, but was mainly around the navel, and all children had symptoms of skin purpura in the course of the disease. The diagnostic sensitivity, specificity and accuracy of NBI endoscopy for HSP of abdominal type were 95.24%(60/63), 71.43%(5/7) and 92.86%(65/70), respectively. Compared with 79.25%(42/53), 29.41%(5/17) and 67.14%(47/70) by abdominal B ultrasound examination, the differences in sensitivity and accuracy were significant (P<0.05).
Conclusion:There are no specific gastrointestinal symptoms in children with HSP of abdominal type. The diagnostic sensitivity and accuracy of NBI endoscopy in children with HSP of abdominal type are high, which can provide a reliable basis for the clinical diagnosis and treatment of children with HSP and the clinical application value is high.
