The etiology of tic disorder in children is complicated, the symptoms are various, and it is easy to be affected by social environment, so the treatment is challenging. Based on many years of clinical experience and through the combination with the theory of TCM and the characteristics of the disease, Professor Ma Rong first established the "Three-stem syndrome differentiation" mode for the treatment of children with tic disorder. The main content is as follows. The first step is disease differentiation, which is to take the root cause as the main point and make clear diagnosis. The second step is syndrome differentiation, which is to classify the syndrome and give prescription based on rules. The third step is symptom differentiation, which is to be clear with the symptoms and make modification. The differentiation of the disease is mainly the diagnosis and differential diagnosis of Chinese and western medicine, and the clinical descriptive diagnosis is used to exclude the symptoms of eye tutting and nose shrugs caused by allergic conjunctivitis and allergic rhinitis. The syndrome differentiation is mainly based on the TCM syndromes of the children, and the main treatment principle is to calm the wind and stop movement. The patients with exogenous wind evil were the syndrome of exogenous wind and were treated with Yinqiao powder with modification. The patients with both beating other people and destroying objects and obscene language were the syndrome of phlegm-fire disturbing the heart, and were treated with Ditan decoction. The patients with irritability were qi stagnation turning into fire, and Xieqing pills were used with modification. The patients who were sensitive with too many doubts and much anxiety were qi stagnation activating wind, and they were treated with Chaihu Guizhi Longgu Muli decoction with modification. Symptom differentiation is based on disease differentiation and syndrome differentiation, and the drugs for treatment are selected according to different parts of twitches and different vocal manifestations, which plays the role of guiding meridians and directly eliminating symptoms. The "Three-step syndrome differentiation" mode has clear diagnosis and treatment plan, and can be adjusted flexibly according to disease-syndrome-symptom of the patients, which is effective in clinical practice.

Professor Wang Xuefeng's experience in treating children with multiple tic disorder based on the five viscera theory

WANG Shuyu, ZHANG Xiuying, WANG Xuefeng.
    2023, 15 (5):  374-378.
    Abstract ( 119 )   PDF (920KB) ( 40 )   Save
The number of children with multiple tic disorder (TD) has been increasing. The incidence of this disease has drawn much social attention due to the special symptoms of child patients, which brings some psychological burden to both children and their parents. The disease mostly occurs in the children aged 5 to 10 years, which has also led to negative impact on the psychological health development of children during their growth process. Many studies have proved that traditional Chinese medicine (TCM) has great effect on this disease because of its advantages of differential treatment, and has much lower rate of adverse reactions than western treatment. It is more likely to be accepted by children and parents, who are more willing to cooperate in the treatment, and it has resulted in great effect. Based on the five viscera theory, Professor Wang Xuefeng believes that the causes and mechanism of multiple TD are closely related to five viscera. According to TCM saying——the wind leading to dizziness belongs to liver, and due to the fact that this disease belongs to the category of liver wind, slow infantile convulsion, spasm, etc. in TCM, Prof. Wang carries out the treatment with liver as the starting point. Based on the TCM theory of viscera, Five-element theory and the theory of five viscera as a whole, and taking clinical symptoms and physiques of children into account, which include the special physiological features (child viscera being delicate, qi being insufficient and development being fast) and the special pathological features (occurring easily, spreading quickly, viscera being clear and recovery being rapid), Prof. Wang thinks that the disease is not only in the liver, but also related to spleen, heart, lungs and kidneys. A brief analysis is made regarding the causes and mechanisms, based on which different treatment principles are decided on, and prescriptions and medications are analyzed.

ObjectiveTo study the medication law of Professor Wang Sumei in the treatment of tic disorders (TD) based on data mining technology in order to provide ideas for the clinical treatment of TD. MethodsProfessor Wang Sumei's TCM prescription data for TD were collected, and the Famous Veteran Chinese Medicine Practitioner Clinical Diagnosis Information Collection System was used to carry out data mining on the selected TCM prescriptions, including drug frequency, efficacy, property, taste and meridian, complex network analysis and so on.ResultsThrough collection and screening, a total of 1 500 case histories were included. The top 5 symptom frequencies of TD were blinking, babbling, shrugging, neck-twisting and frowning. Spleen deficiency and liver hyperactivity was the main type of syndrome. The top 5 herbal medicines were Divaricate saposhniovia root, Fineleaf schizonepeta herb, Pawpaw, Eriocaulon buergerianum, Japanese Clubmoss Herb. As for the efficacy of drugs, anti-wind medicines were used most frequently followed by painkillers and liver-calming medicines. The medicines were mainly warm in property, followed by mild and cold; they mainly pungent in taste, followed by bitter and sweet; they functioned mainly in the liver meridian, followed by the lung and spleen meridians. The drugs with efficacy of clinical control and apparent efficacy in this study were mainly Divaricate saposhniovia root, Fineleaf schizonepeta herb, Eriocaulon buergerianum, Tangerine peel, Pinellia ternata, Pawpaw, etc.ConclusionProfessor Wang Sumei believes that the pathogenesis of TD is spleen deficiency and phlegm aggregation as well as liver hyperactivity and wind movement. The disease is mainly in liver and spleen, and the treatment is mainly to strengthen spleen and calm liver, stop wind and dredge collaterals, which can provide some reference for the clinical treatment of TD.

    2023, 15 (5):  384-388.
    Abstract ( 138 )   PDF (840KB) ( 32 )   Save
Tic disorder is one of the common diseases in pediatrics, and western medicine believes that it may be related to genetic, immune, psychological and environmental factors. Traditional Chinese medicine often categorizes it as diseases such as "liver wind" and "Chi zong". In recent years, with the deepening of researches, the brain gut axis theory has gradually entered everyone's sight. We have found that there is a certain connection between tic disorder and brain gut axis. After connecting the brain gut axis with the liver-spleen correlation in the five viscera correlation of traditional Chinese medicine, and under the guidance of the brain gut axis theory, it is feasible to treat children's tic disorder from the liver and spleen, whether it is decoction, or acupuncture and massage

Professor Ma Rong's clinical approach to the diagnosis and treatment of Tourette disorder in children based on the concept of long-term management
    CHEN Haipeng, MA Rong.
    2023, 15 (5):  398-400.
    Abstract ( 60 )   PDF (795KB) ( 139 )   Save
Professor Ma Rong, based on the concept of long-term management, divides Tourette disorder into three stages: (1) Seizure stage: This stage is characterized by tic disorders, which belongs to the category of "liver wind" in traditional Chinese medicine. The syndrome differentiation is "internal movement of liver wind", and the treatment is mainly "stopping wind and movement". It is prohibited to reduce or stop medication, and it is advised to avoid participating in competitive social activities. (2) Prevention of recurrence stage: At this stage, children are mainly characterized by irritability and it belongs to the category of "liver fire" in traditional Chinese medicine. The syndrome differentiation is "vigorous liver fire", and the treatment is mainly "clearing and purging liver fire". Medication can be reduced appropriately, and it is not recommended to participate in competitive social activities. (3) Stable stage: At this stage, the main characteristic of the children is depression, and it belongs to the category of "liver depression" in traditional Chinese medicine. The syndrome differentiation is "liver depression and qi stagnation", and the treatment is mainly "soothing the liver and relieving depression". The medication can be reduced and stopped, and the child patients can participate in competitive social activities appropriately.

The impact of Buqi Yijing Shengxue Chinese herbal medicines on histone-modifying enzymes in human erythroid cells
CHEN Juexuan, XU Fangwei, LU Zhuoming
    2023, 15 (5):  401-406.
    Abstract ( 105 )   PDF (982KB) ( 50 )   Save
ObjectiveThis study aims to investigate the impact of Buqi Yijing Shengxue Chinese herbal medicines, which are known for their generating blood by tonifying Qi and benefiting essence properties, on histone-modifying enzymes in the human erythroid K562 cell line.MethodsK562 cells were induced with high and low concentrations of several groups of Chinese herbal medicines, including Buqi Yijing Shengxue drugs, Buqi Shengxue drugs and Yijing Shengxue drugs for blood generating, and sodium butyrate was used as a positive control. The mRNA levels of the γ-globin gene were assessed using real-time fluorescence quantitative PCR. Additionally, cellular HDAC1, HDAC2, and HAT enzyme activity levels were analyzed using an enzyme marker by colorimetric detection methods. The mRNA levels of HDAC1, HDAC2, and HAT genes were determined using the PCR method, while protein levels of these genes were determined via Western Blot.ResultsTreatment with Chinese herbal medicine and sodium butyrate significantly increased the expression levels of the γ-globin gene in high-dose Qi-tonifying and essence-benefiting group, high-dose Qi-tonifying group, and low-dose essence-benefiting group. Cellular HAT enzyme activity levels were notably higher in high and low-dose Qi-tonifying and essence-benefiting groups, as well as in high and low-dose essence-benefiting groups than in blank control group, while HDAC1 enzyme activity levels in the sodium butyrate group were markedly lower than those recorded in the blank control group. HAT mRNA levels were notably higher in high and low-dose Qi-tonifying and essence-benefiting groups, as well as in high-dose essence-benefiting group than in blank control group. Western Blot outcomes demonstrated that only the HAT protein levels in the high-dose Qi-tonifying and essence-benefiting group and high-dose essence-benefiting group were considerably elevated compared to those in the blank control group.ConclusionThe molecular mechanism of Buqi Yijing Shengxue Chinese herbal medicines in the induction of γ-globin in gene expression in the treatment of pediatric β-thalassemia may be related to the regulation of the histone-modifying enzyme, which may be mediated by up-regulating the expression of histone acetyltransferase HAT to enhance histone modification in erythroid cells.

Exploring the pathogenesis of attention deficit hyperactivity disorder based on the theory of yin and yang

LI Yaqun, SONG Yuchen, HAN Xinmin, YUAN Haixia.
    2023, 15 (5):  406-409.
    Abstract ( 87 )   PDF (891KB) ( 40 )   Save
Attention deficit hyperactivity disorder(ADHD) is a common psychosomatic disease in childhood. Modern Chinese medicine believes that imbalance between yin and yang is its basic pathogenesis. With the changes of the times, children's diet, emotions and other pathogenic factors have undergone great changes, and yin and yang are constantly changing. In the process, its pathogenesis has developed from a low baseline level of yin and yang imbalance to the current high baseline level of yin and yang imbalance, from yin deficiency resulting in substantial yin deficiency and yang hyperactivity to current yang hyperactivity leading to exhaust of yin liquid. The main clinical syndrome type has changed from yin-deficiency of liver and kidney to excessive heat of heart and liver and inside disturbance of phlegm and heat, and the treatment should focus on the root causes. The first step is to clear the heat of the heart and liver to achieve the balance of yin and yang. By systematic biological method, experimental researches at home and abroad have proved that the relationship between monoamine neurotransmitter dopamine and norepinephrine synthesis, release, and clearance, between dopamine D1-like receptors and D2-like receptors, pro-inflammatory factors and anti-inflammatory factors, which antagonize each other, influence each other, and adjust each other, is closely related to the theory of yin and yang balance in traditional Chinese medicine. At present, the relationship between promoting vesicle circulation factors and inhibiting vesicle circulation factors in dopamine synaptic vesicle circulation is being studied, in order to provide new targets and directions for traditional Chinese medicine treatment of attention-deficit hyperactivity disorder.

Advances in the study of risk factors associated with attention deficit hyperactivity disorder in children

LI Sai, FAN Qiuyue, LIU Xuhua, JIANG Yonghong.
    2023, 15 (5):  410-415.
    Abstract ( 139 )   PDF (1007KB) ( 99 )   Save
Attention-deficit hyperactivity disorder (ADHD) is a most common chronic neurodevelopmental disease in children and adolescents. It is a highly genetic disease, but not all risk factors are genetic. It is estimated that 10% to 40% differences are related to environmental factors, and are the result of the co-action of genetic and environmental factors. At present, the specific pathogenesis is still not clear, but multiple risk factors can lead to the increase of its incidence. Therefore, it is of great significance to the early diagnosis and prevention of ADHD to study the risk factors of its incidence. This paper discusses the association between incidence of ADHD and the following factors: parents' age at pregnancy and mothers being obese before pregnancy, mental disorder of mothers during pregnancy, fever, abnormal thyroid function, preeclampsia and asthma, material exposure during pregnancy and preschool period, low education level of parents, low family income, improper raising and educational methods, and mild brain damage. The aim is to guide the clinicians to reduce exposure and provide references for the later studies. Regarding the above-mentioned high-risk factors, the necessary measures to reduce the incidence of ADHD in children and adolescents include: to strengthen health care during pregnancy, to build harmonious family relationship, to avoid brain function damage and exposure of related materials in preschool period, and to treat primary diseases. ADHD is the result of multiple factors acting together, so it is necessary to manage the incidence of ADHD from multiple aspects.

Research advances in gut microbiota and pathogenesis of pediatric obesity

    2023, 15 (5):  420-425.
    Abstract ( 137 )   PDF (886KB) ( 341 )   Save
With the change of dietary structure and lifestyle, the rate of pediatric obesity is increasing year by year, which not only affects the growth and development, but also leads to the occurrence of metabolic diseases. Therefore, effective prevention and treatment measures should be taken regarding the pathogenesis of pediatric obesity. In recent years, with the development of high-throughput sequencing technology, many studies have pointed out that the structural characteristics of gut microbiota in obese children are different from those in normal children, which is manifested by the increase of relative abundance of Firmicutes and conditioned pathogen as well as the decrease of Bacteroidetes and probiotics. Modern medical science has shown that gut microbiota and its metabolites interact with the host, which can regulate the inflammation response, intestinal homeostasis, energy metabolism, eating behavior and expression of circadian rhythm genes, thus affecting the occurrence and development of obesity. Traditional Chinese medicine believes that the function of spleen is to govern transportation and transformation and is closely related to the gut microbiota in digestion and absorption as well as resistance to evil. When the spleen fails to transport and transform water, the water gathers into dampness and generates phlegm, blocking qi movement, thus affecting the intestinal function and immunity and disturbing the gut microbiota, which promotes the occurrence of obesity. This article reviews the current research progress in pathogenesis of gut microbiota and pediatric obesity, in order to provide new targets for the prevention and treatment of pediatric obesity and improve the clinical effectiveness.

    Abstract ( 72 )   PDF (893KB) ( 15 )   Save
Childhood allergic rhinitis is a common allergic disease in childhood. In recent years, the diagnosis rate of this disease has been increasing year by year, which seriously affects the physical and mental health and quality of life of children. If not intervened early, the disease may induce diseases such as allergic conjunctivitis and bronchial asthma. Traditional Chinese medicine has the advantages of low recurrence rate and no dependence in the treatment of this disease. Based on the theory that "children are pure yang bodies, and their heart is always in excess", Professor Sun Liping believes that evil entering the body is very likely to transform into heat from yang, which can cause heat accumulation in the heart and lungs, so "the syndrome of hidden heat in the lung" can be seen in the clinical syndrome. Therefore, children with allergic rhinitis often show restlessness and dehydration. Emphasis is placed on the use of the "treatment of heart and lungs together" method to treat pediatric allergic rhinitis, and the use of calming and soothing medicine in addition to clearing nasal passages and stopping runny nose has shown clinical efficacy.

ObjectiveTo observe the effect of acupuncture combined with special education in the treatment of autism spectrum disorder(ASD) in children and its effect on their social life ability.MethodsA total of 80 children aged 3 to 6 years with autism spectrum disorder admitted to the Pediatric Encephalopathy Diagnosis and Treatment Rehabilitation Center of the First Affiliated Hospital of Henan University of Chinese Medicine from February 2021 to December 2021 were selected and randomly divided into 2 groups(control group and observation group), with 40 cases in each group, and the score changes of the autism behavior checklist(ABC), childhood autism rating scale(CARS) and normal development of social skills from infant to junior high school children(S-M) and clinical efficacy before and after treatment were compared between the two groups.ResultsAfter treatment, the response rate of 85.0%(34/40) in the observation group was significantly better than that in the control group(65.0%,26/40), with a statistically significant difference(P<0.05).The differences in the improvement of CARS scores, language, communication, self-care ability and social life ability between the two groups are of statistical significance. Acupuncture combined with special education is a better therapy than pure special education(P<0.05).ConclusionAcupuncture combined with special education can effectively relieve the core symptoms of autistic children, especially the language and social ability, improve the ability of social life, relieve family pressure, and enable them to integrate into society as soon as possible, which is conducive to the prognosis of autistic children.

In recent years, due to the influence of social psychological factors, the incidence of psychogenic cough has increased year by year. After a variety of treatment, the symptoms of most of the children with the disease are not improved, or the disease often relapses. Once children have the disease, the symptoms will have a serious impact on their physical and mental health. Professor Wang Yongji, the tutor, believes that the onset of the disease is closely related to the mood of the children. The treatment should be based on the liver, with the principle of soothing the liver and clearing the lungs. The clinical dialectical medication has made up for the disadvantages of long-term ineffective use of antibiotics and cough suppressants in western medicine, and the clinical effect is remarkable.
