Progress in the clinical features and treatment of pediatric heart failure
    ZHAO Qi, SUN Shuna.
    2023, 15 (6):  494-499.
    Abstract ( 155 )   PDF (861KB) ( 185 )   Save
Heart failure is a clinical syndrome caused by structural or functional abnormalities of the heart, which result in limited cardiac contraction and/or filling, and cardiac output cannot meet the body′s demand.Heart failure is one of the common critical diseases in children.Since all organs of children are not yet mature,if heart failure cannot be corrected, all organs are more likely to be damaged, leading to complications and sequelae, which not only seriously jeopardize children′s lives and health, but also bring heavy medical burden to families and society due to difficulty in treatment and poor prognosis. The etiology, pathophysiological characteristics, and treatments of heart failure in children are very different from those of adult heart failure. The main cause of heart failure in children is congenital cardiovascular abnormalities, while other causes include myocarditis, cardiomyopathy, and coronary artery abnormalities.Adrenergic receptor expression levels,miRNA profiles, and expression of genes associated with myocardial abnormalities differ between children and adults. The treatment for heart failure in children includes etiologic treatment,pharmacotherapy,cell-based therapy, and auxiliary device therapies.Treatment and long-term management regimens for heart failure in children are more challenging than those in adults. This article will review the epidemiology, etiology, pathophysiological features, and treatment of heart failure in children.

Professor Wang Xuefeng′s treatment for children with asthma of pneumonia from lungs and intestines based on the theory of "To treat diseases of Zang through managing Fu"
CAI Zhuang, WANG Xuefeng, YANG Jianshu.
    2023, 15 (6):  500-503.
    Abstract ( 116 )   PDF (791KB) ( 6 )   Save
Children are not fully developed in physical shape and qi,and their defence function is weak. When the exogenous evil invades, the lungs are the first to be attacked. Lungs are very likely to be affected by cold or heat,so they are easily damaged and the damage may linger and it is difficult to be managed.Comprehensive Theory on Deficiency and Excess of Essential Questions states that "The diseases of five Zang result from the blocking of six Fu".This article combs Prof.Wang Xuefeng′s experience in treating asthma of pneumonia based on the theory of "To treat diseases of Zang through managing Fu". First, the origin of the theory of "To treat diseases of Zang through managing Fu"(the Theory) is elaborated form the association between physiology and pathology of Zang and those of Fu.Then, based on the pathophysiological features of children, the pathogenic key to the Theory in the development of asthma of pneumonia is that the heat in lungs spreads to the large intestine. Finally, the treatment principles are summarized by further explaining the feasibility of the Theory based on modern researches. The basic principle in the treatment is "Taking away the firewood from under the cauldron",meaning to treat the diseases by managing the root causes, that is,to clear lungs and phlegm and to stop cough and asthma.According to the stool of children, the children′s bowel movement is kept smooth by unblocking Fu in order to expel the evil.By way of treating the diseases of Zang through managing Fu,the Zang and Fu are in good state, the disease course is shortened, and the therapeutic effect is improved,in which way the diseases are prevented form becoming critical. Here a case is provided for verification.

ObjectiveTo study the role of complement C3 and C4 in the early identification of premature infants with early-onset sepsis, and to analyze their predictive value for sepsis complicated with different sites of infection.MethodsThe clinical data of 72 preterm infants with sepsis (sepsis group) and 18 normal preterm infants (control group) were retrospectively analyzed, who were treated in the First Affiliated Hospital of Zhengzhou University from Jan. 2022 to Dec. 2022. The difference in complement levels between the two groups was analyzed. The sepsis group was divided into three subgroups: sepsis with suppurative meningitis group, sepsis with shock group and simple sepsis group. The differences in complement levels between the three subgroups and the normal control group were compared. ROC curve was used to analyze the predictive value of complement levels for infection in the three subgroups.ResultsThere was no significant difference in complement levels between the sepsis group and the normal control group(P>0.05). The level of complement C3 and C4 in sepsis with suppurative meningitis group was higher than that in control group, and the difference was statistically significant (P<0.05). The levels of complement C3 in the sepsis with shock group and the simple sepsis group were lower than those in the control group, and the differences between the two groups were statistically significant(P<0.05). The levels of complement C4 in the sepsis with shock group and the simple sepsis group were lower than those in the control group, but the difference was not statistically significant(P>0.05). The predictive value of complement level for sepsis combined with shock and sepsis alone was high, but the predictive value for sepsis combined with suppurative meningitis was not high.ConclusionComplement levels are significantly different in children with sepsis complicated with different sites of infection or at different stages of the same infection. Dynamic detection of complement levels may be helpful for the early diagnosis and differential diagnosis of sepsis and its corresponding complications in preterm infants.

ObjectiveTo explore the clinical application of behavior training based on trans-theoretical model (TTM) in the rehabilitation of children with epilepsy through single-blind design.MethodsA total of 95 children with epilepsy admitted to the pediatric Department of the First Affiliated Hospital of Xinxiang Medical College from January 2021 to December 2022 were randomly divided into observation group (48 cases) and control group (47 cases). The control group was given routine health behavior training including medication management, healthy lifestyle and interpersonal communication. The observation group received behavioral training intervention based on TTM in addition to the treatment for control group; compare the total effective rate of rehabilitation, emotional state (children's anxiety mood disorder scale score and children's depression disorder self-rating scale score), self-esteem level (self-esteem scale score), adaptive behavior (independent functional factors, cognitive functional factors and social/self-control factors) of the two groups.ResultsThe total effective rate of rehabilitation in the observation group was 91.67% (44/48), higher than that in the control group (76.60%, 36/47), and the difference was statistically significant (P<0.05). After intervention, the scores of the anxiety mood disorder scale and the self-rating scale of depressive disorder in children in the two groups were significantly lower than those before intervention, and the observation group was lower than the control group; the score of self-esteem scale was significantly higher than that before intervention, and the observation group was higher than the control group, and the difference was statistically significant (P<0.05). The adaptive behavior scores of the two groups after intervention were significantly higher than those before intervention, and the observation group was higher than the control group (P<0.05).ConclusionBehavioral training based on TTM is effective in the rehabilitation of children with epilepsy, which can effectively improve their emotional state, self-esteem level and social adaptive behavior.

ObjectiveTo explore the impact of applying behavioral training (ABA) combined with the "60-point mother theory" on the rehabilitation treatment of children with autism.MethodsTotally 62 children with autism admitted to the Rehabilitation Center from June 2020 to January 2022 were selected as the research subjects. According to the principle of matching basic features between groups, they were divided into a control group and an observation group, with 31 cases in each group. The control group received comprehensive rehabilitation treatment, with ABA training as the main intervention. The observation group received ABA training combined with the "60-point mother theory" as an auxiliary guide. The autism behavior of the two groups of children before treatment and three months after treatment were compared by Autism Behavior Checklist(ABC). The Psychological Education Scale (PEP-3) for autism spectrum and related developmental disorders was used to make a comprehensive evaluation of the children with autism.ResultsAfter 3 months of intervention, the improvement in PEP-3 scores in the observation group was significantly better than that in the control group(P<0.05).The total ABC scores of the two groups were lower on discharge than on admission, and the observation group was lower than the control group(P<0.05).ConclusionThe use of ABA training combined with the "60-point mother theory" is of great significance in children with autism, as it can effectively improve the overall developmental functional level and alleviate clinical symptoms of children with autism.
