Functional constipation (FC) is a common gastrointestinal disease in children. The global prevalence of functional constipation in children is about 0.5% to 32.2%,which seriously affects the quality of life of children.Wan Quan is a famous pediatric doctor in ancient China, who innovatively put forward the idea of "spleen insufficiency" in children, and paid attention to regulating the spleen and stomach, which has an important guiding role in the diagnosis and treatment of modern pediatric diseases.Wan Quan put forward the general principle of the treatment for pediatric constipation in "Infant Family Secrets:Volume Ⅳ:Urination and Defecation Diseases":"The stool induration should be attacked, but sometimes is attacking method useless in spite of the condition".Based on Wan Quans theory of "spleen insufficiency",and "The stool induration should be attacked, but sometimes attacking method is useless in spite of the condition",and combined with modern research progress,it is proposed that the causes of FC in children are mostly attributed to dryness-heat accumulation, stagnation of milk and food, spleen deficiency leading to transportation failure,qi stagnation, and blood deficiency resulting in intestinal dryness.Spleen deficiency leading to food injury is considered to be the key pathogenesis;therefore,it is recommended that the treatment for FC in children should differentiate deficiency from excess, and carry out the principle of activating spleen to assist the transportation throughout the treatment process. This study explores the differentiation and treatment strategies for FC in children from the aspects of acute purgation, digestion and elimination of accumulated stagnation, slow purgation, regulating qi,and moistening dryness, etc.,in order to provide a reference for the clinical diagnosis, treatment, and care of FC in children.

Functional constipation in children is a common disease in pediatrics, which is mainly manifested as difficulty in defecation, reduced frequency of defecation and/or prolonged defecation. The incidence and recurrence rate of this disease is relatively high, and at the same time, functional constipation seriously affects the quality of life of children, which is unfavorable to the growth and development of children. In recent years, with the increase of social pressure, the incidence of this disease has been increasing year by year;in addition to physical symptoms, children′s mental symptoms also gradually gain concern, and very often functional constipation is accompanied by abnormal emotional symptoms in children. The mechanism of functional constipation is mainly "abnormal interaction between the brain and intestines" as proposed in Rome ,which is also related to the holistic concept of "five organs as one" and the theoretical view of form and spirit being all as one in Chinese medicine,so this article takes the theory of "brain-intestine correlation" and the "brain-intestine axis" in modern medicine as the entry point to make an in-depth analysis of the doctrine of "brain-intestine correlation".Based on the special connection between the brain and the intestine, which are physiologically related to each other and pathologically influenced by each other,we believe that the treatment for functional constipation should not only regulate the gastrointestinal function to improve intestinal symptoms, but also pay attention to the role of the heart and brain in regulating the spirit to improve the patients anxiety, depression and other mental symptoms, and explore the treatment method of regulating the spirit to achieve the effect of intestinal transport, aiming to find a new way of thinking about the treatment for functional constipation, and provide a new method to improve the quality of life of children with functional constipation.

ObjectiveTo report the diagnosis and treatment process of a patient with Alstrom syndrome to improve clinical understanding of this disease.MethodsAnalyze the clinical manifestations, laboratory tests, diagnosis, and treatment process of a patient with Alstrom syndrome admitted to our hospital in March 2023, and discuss them in combination with domestic and foreign literature. ResultsThe first symptom of the patient was nystagmus, and then the patient gradually appeared decreased vision and hearing, acanthosis nigricans, diabetes, hypertriglyceridemia, liver dysfunction, urinary protein, fatty liver, subclinical hypothyroidism, and candida albicans infectious vaginitis, etc.. The genetic results showed that there were two mutations in the ALMS1 gene, both of which were located in exon 8. The ALMS1 gene c.6316C>T(p.Gln2106Ter) heterozygous mutation was a nonsense mutation, resulting in the termination of protein coding at the 2 106th position of glutamine; ALMS1 gene c.4642_4658del(p.Arg1548Trpfs*4) heterozygous mutation, a frameshift mutation, resulted in the protein encoding arginine being replaced by tryptophan at the 1 548th position , and premature termination of the codon at position 3,which was ultimately diagnosed as Alstrom syndrome. This disease has been currently incurable and most treatments were symptomatic. ConclusionAlstrom syndrome is rare in clinical practice, with complex and diverse manifestations, which can easily lead to misdiagnosis and missed diagnosis. For diabetes patients with impaired vision and hearing, we should be alert to Alstrom syndrome, and carry out gene detection as early as possible to make a clear diagnosis and early intervention to improve the quality of life of patients.

    2024, 16 (1):  58-62.
    Abstract ( 83 )   PDF (1055KB) ( 242 )   Save
ObjectiveAnorexia nervosa in children is a common pediatric condition, which usually manifests as a long-term decrease in appetite or even refusal to eat. Anorexia can lead to imbalance in growth and development and a series of systemic diseases. Therefore, the prevention and treatment of anorexia nervosa has become a hot topic in Chinese and Western pediatric researches. Tui Na is a method of treatment which promotes the circulation of Qi and blood and regulates Yin and Yang by stimulating acupoints with Yin-Yang and Five Elements and meridian theory as the theoretical basis and differentiated treatment as the principle. This article introduces the clinical thinking and acupoint selection experience of Mr. Wang Huiyao, former director of the Second Affiliated Hospital of Guiyang College of Traditional Chinese Medicine, in the treatment of anorexia nervosa in children with Tui Na. According to the primary etiology and pathogenesis of anorexia nervosa being "disharmony between spleen and stomach and loss of normal functioning of spleen", and based on years of experience in managing pediatric diseases and observation on children being unable to talk about their disease clearly, Prof. Wang uses the method of "running through up and down and regulation between before and after" in clinical selection of acupoints to promote the digestion, absorption, excretion and transfer function of spleen and stomach, in order to regulate Yin-Yang balance and organ functions, which achieves the purpose of stimulating the children's own potential and their self-healing ability and improving clinical effect in the treatment of anorexia nervosa. After decades of standardized research on pediatric Tui Na therapy for anorexia nervosa, through data mining and clinical selection of acupuncture points, he summarized the basic therapeutic prescription for anorexia nervosa, which better improves the efficacy of Tui Na therapy for anorexia nervosa and avoids the pain of acupuncture and medicine for children. Attached is a case study of anorexia nervosa in pediatricpatients treated with the basic formula of anorexia nervosa by Mr. Wang, which can be used for reference and discussion among colleagues so as to provide better service to the children.

Professor Wang Xuefeng's experience in treating refractory nephritic syndrome combined with atopic dermatitis in children
SHEN Xiaoyu, WANG Cuijin, WU Yan, YIN Lei, SUN Kexing, SHA Sha, WANG Xuefeng.
    2024, 16 (1):  72-76.
    Abstract ( 65 )   PDF (1055KB) ( 217 )   Save
Refractory nephrotic syndrome(RNS) is difficult to treat and has a high recurrence rate. Modern research has found that patients are often accompanied by an increase in total IgE in peripheral blood, which has many similarities with atopic diseases. It is speculated that the pathogenesis and cytokine changes are similar between RNS and atopic dermatitis(AD). The clinical treatment for RNS combined with AD is more challenging. Professor Wang Xuefeng, based on the theory of the Huangdi Inner Canon, proposes that there are three phases of "powerful fire inside", "powerful fire consuming qi" and "junior fire insufficiency" in the corticosteroid therapy for RNS. She believes that the key pathogenesis of steroid-dependent(SDNS) and frequently relapsing nephrotic syndrome(FRNS) is "powerful fire consuming qi", and the focus in the treatment of SDNS/FRNS is to maintain the function of junior fire generating qi, and to reduce the side effects of "powerful fire consuming qi", so as to prevent steroid dependence and frequent recurrence. Also, RNS combined with AD is different fromcommon sores and furuncles; it is Yin deficiency, body weakness and toxin strength. After the lesion is removed, the toxin has not gone and Yin becomes less, so the disease is difficult to heal. Professor Wang Xuefeng emphasizes that different medications should be used for different stages of NS, and she pays attention to Qi nourishing and Yin nourishing, which had a good clinical effect.

ObjectiveTo analyze the medication rules of national patent Chinese herbal compound prescriptions for eczema and atopic dermatitis based on data mining in order to direct the development of new medicines and basic researches.MethodsWe searched for Chinese herbal compound patents for the treatment of eczema and atopic dermatitis in the State Intellectual Property Office's patent database, and we statistically analyzed those patents using the bibliometric approach, data mining analysis, association rule analysis, etc.. Using the TCM Heritage Assist Platform V2.5, we created association rule network diagrams using SPSS Modeler 18.0 and made cluster analysis using SPSS 26.0.ResultsThis study examined 380 herbal medicines used in a total of 425 internal Chinese herbal compound formulas for the treatment of eczema and atopic dermatitis. All of the medicines were bitter and cold in nature, with Kushen, Difuzi, Huangbai and Baixianpi serving as the main ingredients; the majority of them functioned through the liver, stomach, lung, and spleen meridians. Further investigation into potential new prescriptions was made.ConclusionThe fundamental principle of "clearing heat and dampness, dispelling wind and relieving itch" is summarized as the main principle of clinical application of TCM for eczema and atopic dermatitis, and data mining provides new theoretical foundation for clinical prevention and therapy.

To discuss Professor Sun Liping's clinical experience in treating pediatric mesenteric lymphadenitis. Professor Sun Liping believes that the disease is concerned with the spleen and stomach; the internal cause is the deficiency of spleen and insufficiency of pediatric-yang, and the external cause is the injury caused by cold weather, excessive consumption of cold food and medicine. The disease is treated in two phases: the exacerbation phase and the remission phase. In the exacerbation phase, the treatment principle is to soften hard masses and resolve stagnation, and to warm the spleen and stomach and stop pain; it is advisable to use Tianzhu powder with modification to eliminate and supplement, mainly to eliminate. In the remission phase, the treatment principle is to replenish the spleen and strengthen qi, and to warm the spleen and stomach and tonify deficiency, with tonification being the mainstay; it is advisable to use Shenling Baizhu powder with modification.

A brief analysis of the pathogenesis and syndrome differentiation of autism spectrum disorders from the "brain-spleen and kidney correlation"
CHEN Xi, PENG Yaqi, LI Wei, YANG Hongsheng, WANG Junhong.
    2024, 16 (1):  88-92.
    Abstract ( 115 )   PDF (1055KB) ( 576 )   Save
Autism spectrum disorders(ASD) is characterized by sustained impaired social ability, repetitive behaviors or interests, and stereotyped behavior. It is a common pediatric neuropsychiatric disease with great harm in TCM pediatrics, and in 2020, the domestic incidence has reached 7‰. The optimal treatment period is often missed due to delayed diagnosis, limited treatment measures, failure to adhere to treatment, improper parental care and so on. The "Original Spirit" of ASD is damaged, and the lesion is in the brain. This paper analyzes the correlation between brain and spleen and kidney from the material basis, structural basis, operational basis and pathological connection, and concludes that the deficiency of spleen and kidney is the fundamental pathogenesis of ASD, and phlegm and blood stasis is the important pathological factors of ASD. Professor Wang Junhong thinks that the physiological and pathological characteristics of children should be considered clinically, and the basic treatment should be to tonify the kidney and spleen, eliminate phlegm and remove blood stasis. At the same time, there should be no improper tonification, and the focus is to regulate the whole body movement and chemical function of children. In the treatment, the classic prescription Liuwei Dihuang pills and Liujunzi decoction are selected as the main prescription of Bushen Yunpi Yizhi prescription. According to the viscus syndrome, the treatment combines comprehensive analysis of the four examination methods and achieves good effects. Based on clinical cases, this article introduces the key points of syndrome differentiation, treatment and medication experience of Bushen Yunpi Yizhi prescription in the treatment of children with ASD.
